backgrounds: the aim of this study was to determine if preemptive local anesthesia yields better postoperative pain control than infiltration of local anesthesia at the time of wound closure. methods: forty patients aged between 1 and 10 years were randomly allocated to one of the two groups by using a sealed envelope technique. group 1 received 0.5 mg/kg bupivacaine 20 minutes before the incision of skin and the same volume of normal saline at the end of skin suture. group 2 received 0.5 mg /kg bupivacaine at the end of skin suture and the same volume of normal saline 20 minutes before the incision of skin. pain scores of patients in the recovery room, 6, 12 and 24 hours after surgery were measured. if patients complained of post surgical wound pain, 30 mg/kg of acetaminophen was administered by rectal suppository. data were analyzed by chi-square test, t -test and anova. results: there were no statistical significance between the two groups for age, weight and sex. the overall mean of pain was 4.6 ± 2.6 for group 1 and 18.6 ± 8.7 for group 2 and the difference between the two groups was statistically significant (p<0.001). the mean dosage of acetaminophen administration was significantly higher in group 2 compared with group 1 (p<0.05). conclusions: pre-surgical infiltration of bupivacaine in the surgical field is a useful method in decreasing both post-surgical wound pain for up to 24 hours and analgesic consumption after inguinal hernia repair.

The Effect of Pre-Incision Field Block versus Post-Incision Inguinal Wound Infiltration on Postoperative Pain after Paediatric Herniotomy

BACKGROUND The Ilioinguinal/iliohypogastric nerve block has been shown to significantly decrease opioid analgesic requirements and side effects after inguinal herniotomy. We compared the effect of pre-incisional field block with 0.25% bupivacaine and post-incisional wound infiltration with 0.25% bupivacaine for postoperative pain control after inguinal herniotomy. PATIENTS & METHODS This was ...

The effect of Pre-incision skin infiltration with Lidocaine on postoperative pain following abdominal hysterectomy

Introduction: Several mehods have been proposed to alleviate pain after hysterectomy. Pre-emptive analgesia has been used to relieve pain following abdominal hysterectomy with conflicting results. This study was performed to evaluate the efficacy of pre-incision skin infiltration of Lidocaine in relieving postoperative pain in patients undergoing abdominal hysterectomy.  Methods: 60 patient...

Effects of Suppository Acetaminophen, Bupivacaine Wound Infiltration, and Caudal Block With Bupivacaine on Postoperative Pain in Pediatric Inguinal Herniorrhaphy

BACKGROUND The control of postoperative pain is important in children, and poor pain control leads to organ dysfunction and behavioral problems. OBJECTIVES We compared the analgesic effects of suppository acetaminophen, bupivacaine wound infiltration, and caudal block with bupivacaine on postoperative pain in pediatric inguinal herniorrhaphy. PATIENTS AND METHODS In this double-blinded, ran...

effects of suppository acetaminophen, bupivacaine wound infiltration, and caudal block with bupivacaine on postoperative pain in pediatric inguinal herniorrhaphy

conclusions patients in the bupivacaine infiltration and caudal block groups had less postoperative pain than those in the acetaminophen group and received lower amount of meperidine. we concluded that in children, bupivacaine infiltration and caudal block with bupivacaine produce better analgesia than suppository acetaminophen. it seems that bupivacaine infiltration is better than caudal block...

Chronic postoperative pain: the case of inguinal herniorrhaphy.

That surgical injury can lead to chronic pain is now well established. 21 29 41 50 From these reviews and studies using a systematic collection of data, the estimated incidences of chronic pain after various procedures are: leg amputation about 60%, thoracotomy about 50%, breast surgery about 30%, cholecystectomy 10–20%, and inguinal herniorrhaphy about 10%. Predictive risk factors for chronic ...

Transitioning to Single-Incision Laparoscopic Inguinal Herniorrhaphy

BACKGROUND Laparoendoscopic single-site surgery (LESS) offers cosmetic benefits and may represent further progress towards reducing the invasiveness of surgical interventions. We report our initial experience with LESS totally extraperitoneal (TEP) inguinal herniorrhaphy. MATERIALS AND METHODS Beginning March 2009, we transitioned from a multiport laparoscopic TEP (MLH) technique to a single-...
